Product Condition:
For full transparency, this disc was probably wiped by the previous owner. In other words, the audio for the original cleaning track was not present.
I re-recorded the audio from a different lens cleaning disc onto this disc to recreate audio for the cleaning track.
This does not affect the lens cleaning capability in any way, as the physical brushes that clean the lens are unaffected (the reading lens reads from the under-side of the disc).
Furthermore, write head cleaning is unaffected as the write head makes contact with the top surface of the disc. Recording additional tracks will clean the writing head as expected.
Tested and confirmed the disc is readable when inserted into a MiniDisc player/recorder.
Almost no signs of use.

Instructions for Use (lens cleaning operation):
1) Place the disc in the MiniDisc player/recorder, and wait for it to read the disc contents.
2) Press the play button to play track #1.
3) Wait for the track to finish playing.
4) Cleaning is completed. 
5) Remove the disc, and place in its case.

Instructions for Use (recording head cleaning operation):
1) Place the disc in the MiniDisc recorder device, and wait for it to read the disc contents.
2) If necessary, set the recorder so that it begins recording from the end of any existing tracks.
3) Begin recording from any source.
4) Stop recording after 10 seconds. Recording head cleaning is completed. 
5) Remove the disc, and place in its case.
