Smith & Nephew Iodosorb is a sterile 0.9% cadexomer iodine antimicrobial wound gel — the tube-delivery format of the same clinically proven cadexomer iodine technology used in Iodoflex pads — designed for applying directly into wound cavities, irregularly shaped wounds, and wound areas where a pad dressing cannot achieve full contact with the wound base. When applied, Iodosorb absorbs exudate, slough, pus, and debris into its unique cadexomer matrix at a rate of up to 7 times its own weight — releasing iodine in a sustained, controlled manner as it absorbs, providing broad-spectrum bactericidal activity for up to 72 hours per application. The cadexomer matrix has been shown to physically disrupt mature biofilm — absorbing it into the dressing and reducing bacterial load within the biofilm — a critical differentiator for chronic wounds stalled by biofilm. As the iodine is progressively consumed, Iodosorb changes color from its characteristic brown to white — a clear, unambiguous visual indicator that tells clinicians and caregivers precisely when it is time to change the dressing, eliminating guesswork and supporting consistent treatment intervals.

Indicated For — Wound Types

  • Venous stasis ulcers — wet, exuding, and infected
  • Pressure sores and pressure injuries — all stages with exudate
  • Diabetic foot ulcers — infected or heavily colonized
  • Infected traumatic wounds
  • Infected surgical wounds
  • Chronic exuding wounds with slough, pus, debris, or biofilm
  • Wounds with confirmed or suspected MRSA or Pseudomonas aeruginosa infection
  • Any wet wound requiring simultaneous antimicrobial activity, desloughing, and exudate management

Important Contraindications: Do not use on dry wounds — Iodosorb is not effective when there is insufficient exudate to activate the cadexomer matrix. Not for use in patients with known or suspected iodine sensitivity, Hashimoto's Thyroiditis, Grave's Disease, or non-toxic nodular goiter. Not for use by pregnant or lactating women. Not for use in children 14 years and under. Use with caution in patients with severely impaired renal function or history of thyroid disorder. Do not use continuously for more than 3 months. Potential interaction with lithium — increased risk of hypothyroidism. Do not use concomitantly with mercurial antiseptics or taurolidine.

As the beads absorb, iodine is released at a controlled, sustained concentration — bactericidal without the cytotoxic spike of free iodine solutions — maintaining effective antimicrobial activity for up to 72 hours. The gel formed over the wound surface creates a moist environment that supports autolytic debridement and granulation tissue formation. Remove by gently rinsing with sterile saline or water — moisten if any material is tending to adhere before removal. Do not exceed 50g per single application or 150g per week.

I had the privilege of working with Otis during part of his training at the National Cancer Institute at a time that policies on PSA and mammography screening were being newly scrutinized with respect to risk/benefit. The notion that screening might actually do more harm than good was seen as absolute heresy and I fondly remember the wonder and amazement in Otis' eyes as he unraveled to me just how this counter-intuitive idea could be so. I am at a personal watershed and this book is a strong antidote for what ails me, i.e. I feel emboldened to take my career in the direction of a solution rather than continue to contribute to the problem. Written for the general audience, We Do Harm breaks the ranks of legions of doctors who are invested in perpetuating a broken but remunerative healthcare system by introducing specific and by no means isolated instances where patients have been harmed. This book is at once disparaging of blind trust of one's physician and optimistic in that some stars like Otis and microcosms like Grady Memorial can and do avoid patient-disadvantaged conflicts of interest. This book provides evidence and motivation for every person in every role they engage in in the healthcare system: patient, consumer, provider, insurer, politician. No one can escape the relevance and import of the key message of this book that "more" is not always "better." Until such time, if ever, that our healthcare system operates under transparency, this is a must read for all. That means you! Now! It can't wait! I am a believer that healthcare quality will come from the bottom up and not the top down and this book exemplifies this spirit; read it and be empowered to ask the hard questions as to who is motivated to provide what care and at what personal gain.
