IMPORTANT: Please fill out all forms in the below questionnaire. You will be able to check out with the membership temporarily at $0. We will review the questionnaire and get back to you via the e-mail you used to check out to either confirm or deny your membership. If confirmed, you will be charged the normal monthly membership fee of $125 the next day. If denied, your "membership" will be cancelled, and you won't be charged.
